Registered Social Worker BayWest Medical Centre ? Ajax, Ontario BayWest Medical Centre is seeking a Registered Social Worker to join our growing healthcare team. The successful candidate will provide compassionate, patient-centred support to individuals and families experiencing a range of social, emotional, and life challenges. Position responsibilities * Conduct psychosocial assessments and identify patients? social and emotional needs * Provide individual counselling and supportive interventions * Support patients experiencing stress, anxiety, grief, family concerns, and other psychosocial challenges * Assist patients with navigating community resources and social services * Provide support with housing, financial, employment, and other social needs where appropriate * Develop individualized care plans and provide appropriate follow-up * Collaborate with physicians and other healthcare professionals as part of a multidisciplinary team * Provide crisis support and appropriate referrals when required * Maintain accurate and timely documentation in the EMR * Maintain patient confidentiality and adhere to professional and ethical standards Qualifications * Registered Social Worker in good standing with the Ontario regulatory body * Bachelor?s or Master?s degree in Social Work * Previous experience in a healthcare, primary care, or community setting is an asset * Strong counselling, communication, and interpersonal skills * Ability to work independently and collaboratively within a multidisciplinary healthcare team * Experience with electronic medical records (EMR) is an asset * Compassionate, professional, reliable, and patient-focused Position details Location: BayWest Medical Centre, Ajax, Ontario Position: Registered Social Worker Employment type: Full-time How to apply Please email your resume to baywestajax@gmail.com with ?Social Worker? as the subject line.
