About the Opportunity We are seeking a licensed Pharmacist to join a growing clinical team focused on improving patient outcomes through medication management and patient engagement. This is a remote position that combines direct patient interaction, clinical decision-making, and collaboration with providers and pharmacies to support quality healthcare initiatives. Key Responsibilities Conduct comprehensive medication reviews and provide medication counseling to patients. Support medication adherence initiatives and identify opportunities to optimize therapy outcomes. Collaborate with providers, pharmacies, and healthcare teams to address patient concerns and coordinate care. Review patient information, document clinical interventions, and maintain accurate records. Manage a consistent volume of patient outreach while meeting quality and performance expectations. Contribute to process improvement initiatives and support team collaboration in a fast-paced environment. Assist with onboarding and mentoring team members as needed. Qualifications PharmD degree and active Pharmacist license. Strong clinical knowledge across chronic disease states and medication management. Experience engaging patients and providing education through phone-based or virtual interactions. Excellent communication, documentation, and organizational skills. Ability to work independently while managing multiple priorities in a metrics-driven environment. Comfortable learning new technologies and navigating multiple systems simultaneously. Preferred Experience Medication Therapy Management (MTM) experience. Managed care, PBM, health plan, ambulatory care, or chronic disease management experience. Experience working with Medicare populations and quality-based healthcare programs. Previous remote or virtual healthcare experience. Additional state licenses, board certifications, or residency training are a plus. Work Environment This is a full-time remote opportunity with a standard weekday schedule and occasional Saturday coverage based on business needs.
