Clinical Pharmacist – Medication Therapy Management Position Summary We are seeking an experienced Clinical Pharmacist to support a growing Medication Therapy Management (MTM) initiative focused on comprehensive medication management and optimizing clinical outcomes. This role will work closely with internal clinical pharmacy teams and external partners to review algorithm-generated medication interventions, conduct provider outreach, and support evidence-based prescribing decisions. The ideal candidate is a self-motivated clinical pharmacist with strong communication skills, experience evaluating medication therapies, and the ability to work independently in a remote environment. Key Responsibilitie sReview medication-related intervention opportunities identified through clinical algorithms .Evaluate the clinical appropriateness of prescribed medications, including dosage, duration of therapy, and other medication management considerations .Conduct outreach to healthcare providers to discuss medication-related recommendations and gather additional clinical information .Communicate evidence-based clinical recommendations that support optimal patient care and prescriber decision-making .Collaborate with pharmacists, project managers, business analysts, and other stakeholders to support ongoing program implementation and growth .Document outreach activities and intervention outcomes accurately and efficiently .Utilize Excel to sort, filter, analyze, and manage data sets related to intervention opportunities and provider outreach activities .Support the continuous improvement of medication management programs through clinical feedback and operational insights .Required Qualification sActive Pharmacist license, preferably in Pennsylvania .Minimum of 5 years of clinical pharmacy experience .Strong understanding of medication therapy management, medication utilization review, and clinical pharmacy practices .Proficiency in Microsoft Excel, including data sorting, filtering, and manipulation of large datasets .Excellent written and verbal communication skills .Ability to work independently with minimal supervision .Strong organizational skills and the ability to manage multiple priorities in a largely autonomous role .Preferred Qualification sExperience with academic detailing or provider-facing clinical outreach programs .Experience reviewing medication utilization data and providing evidence-based recommendations to prescribers .Background in comprehensive medication management, population health, managed care, or related clinical pharmacy programs .Retail pharmacy experience will be considered when combined with the required clinical and communication skills .Education & Licensur eActive Pharmacist license in state of Pennsylvania required .
