Full Time Position: Case Manager Neil Squire Society, Burnaby, BC Neil Squire Society is a Canadian national not-for-profit organization that empowers Canadians with disabilities through the use of computer-based assistive technologies, research and development, and various employment programs. Headquartered in Burnaby, BC, Neil Squire is presently hiring for their employment program and seeking a Case Manager. Under the direction of the National Program Manager, the Case Manager is responsible for conducting intake assessments, providing one-to-one employment counselling sessions, developing action plans with program participants, and providing all aspects of administrative support needed to the national employment program. Duties and Responsibilities: * Respond to phone calls and emails from individuals and service providers seeking information on the program. * Screen clients for eligibility/suitability for the program, conduct intake assessments, and initial needs determination with clients. * Manage all aspects of client action plans and refer clients to other services as appropriate. * Keep in contact with clients and team members. Enter program notes, job development activities and employer activities into the appropriate databases. * Verify and ensure accuracy of all database entries made by BC and regional staff and submit entries on the portal system (new client, interventions, closures, and follow-ups). * Review all BC and regional client files, including intake forms, interventions, and closures, to ensure the required documentation is included in physical and electronic files. * Review reports to ensure accuracy and compile the reporting as needed. Assist Burnaby, BC, staff with compiling and submitting reports. * Generate weekly reports using the database reports and send them to the team. * Submit Assistive Technology referrals to the Solutions team and update the database/client file when the AT Assessments are ready. * Complete all administrative work-related tasks, including forms management, client file management, and other work assigned, ensuring all Excel trackers and other documentation used are kept up to date. * Maintain filing systems and electronic folder systems. * Under the direction of the National Program Manager, communicate with Burnaby, BC, staff as well as distant BC staff on certain employment-related program or client matters; communicate with regional staff as required. * Ensure all statistics, case notes, trackers, reports, consents, and other information pertaining to client documentation are completed on time and maintained in adherence to policies and procedures. * Respond to queries from the Assistive Technology Services program and refer self-referrals to the provincial government online employment services portal. * Other additional duties will be assigned as needed. Education, Training, Experience and Other Requirements: * Relevant Bachelor?s Degree, Diploma in Social Sciences or a related field and/or CDP Certificate OR an equivalent combination of education, training and experience is an asset. * Willing to undergo a Criminal Record Check is a requirement. * Must have strong administration skills and a high level of computer proficiency including Microsoft Office Suite. * Ability to prioritize multiple duties requiring scrupulous attention to detail. Confidentiality, professionalism and prompt action are mandatory. * Demonstrated ability to meet deadlines and work under pressure is a requirement. * Excellent communication (verbal & written), interpersonal, organizational and time management skills are necessary.
