Chippewas of the Thames First Nation is accepting applicants for the position of: Position Title: Community Health Nurse Department: Health Department Hours of Work: 37.5 hours per week working onsite 8:30am ? 4:30pm *Location: *320 Chippewa Road, Muncey ON Posting Date: July 17, 2026 Posting Closes: August 9, 2026 Summary: The Community Health Nurse (RPN/RN) is responsible for identifying, assessing, planning, implementing, delivering, and evaluating Community Health Programs under the direction of the Community Health Nurse Supervisor. This role actively supports community health development and strategic planning efforts identified in the Community Story and Comprehensive Community Plan, contributing to holistic health programming aligned with the social determinants of health to address the diverse health needs of residents of Chippewas of the Thames First Nation. In this role, you will consult and collaborate with individuals, families, community partners, other professionals, and government partners to promote and preserve community health through health promotion, health maintenance, and health education. Key efforts include delivering mandatory Communicable Disease and Immunization programs while maintaining accredited standards (Canadian Accreditation Council) and current practice knowledge through the First Nations and Inuit Health Branch (FNIHB). Responsibilities: * Collaborate with the Community Health Nurse Supervisor and Health Department staff to plan, deliver, and improve nursing programs and services. * Apply the nursing process (assessment, care planning, implementation, evaluation) with individuals/families, including those at risk or requiring nursing supports. * Provide comprehensive community health nursing services through clinic visits, home visits, school visits, and planned classes/workshops. * Deliver and coordinate communicable disease control and immunization programs, including required reporting, documentation, and maintaining/refreshing related plans (e.g., pandemic planning). * Plan and support community wellness initiatives, prevention initiatives, and community awareness campaigns. * Provide geriatric-focused supports, including outreach/assessments, referrals/placements, and maintenance of registers. * Provide nursing support and referrals for mental health needs, coordinate care with community agencies, and provide information/support to families. * Provide chronic disease supports, including assessment, coordination of home-based care, and healthy lifestyle education (e.g., diabetes education supports and referrals). * Support care coordination, facilitate assessments/referrals, and collaborate with families, schools, and agencies for children and adults with physical, developmental, or psychological/behavioural concerns. * Deliver and/or coordinate health education programming (e.g., childbirth education, family planning, nutrition, physical activity, smoking cessation, chronic disease prevention). * Support environmental health activities, including water sampling coordination, follow-up to water testing, and community education regarding environmental contaminants. * Maintain accurate, timely client records, registers, consent documentation, and reporting (monthly/annual outcomes) in departmental systems (e.g., Path DMS). * Maintain confidentiality and participate in accreditation activities, quality improvement standards, audits, and required program reporting. * Promote a safe workplace by following safety rules, reporting hazards, and participating in meetings, committees, and cross-department collaboration. Qualifications: * Completion of a university, college, or other approved nursing program (RN/BScN or RPN). * Valid Certificate of Registration with the College of Nurses of Ontario. * Minimum three (3) years of experience in Community Health (preferred). * FNIH-approved training in Community Health, and membership with the Registered Nurses' Associati
