The primary goal of HOMEBASE is to support communities in connecting youth to the right services, in the right place, and at the right time, with a strong emphasis on collaboration, multidisciplinary care, and early intervention.The Community Paramedic (CP) plays a key role in promoting health and preventing illness and supplements existing Primary Health Care services. This role is responsible for identifying health needs and risks, developing and implementing health programs, providing direct patient care, and collaborating with other healthcare professionals to improve access to care and health outcomes for youth in the community.As part of the Mobile Integrated Health Team, the Community Paramedic works closely with YMCA Homebase staff and supports a diverse patient population, including youth requiring mental health and addiction services, physical health supports, and coordination of care and community-based services.
