Title of Job: Case Manager Date: July 21, 2026 Closing Date: August 4, 2026 Location: Toronto, ON - Mount Dennis ICM Compensation: $57,007 - $71,647 Annually Prorated Contract Duration: 6-Month Contract Work Type: Onsite Job ID: 10924966 Existing or New Position: Existing Program Summary: Mount Dennis ICM provides specialized care coordination and one-on-one support for adults and seniors with mental health and addiction challenges. Our Case Managers support clients by facilitating recovery. Our program also supports the entire building of Toronto Community Housing tenants with community development engagement and activities. These can range from bringing in health education and resources to celebrating holidays with special meals and events. The goal is to create a vibrant, healthy community within the entire building. Job Summary: As a Case Manager you will work independently or be part of a team that co-ordinates a support network and links clients living with mental illnesses, addictions and physical health issues. The position attempts to empower the client, thereby increasing their independence, dignity and self-respect in a safe and secure environment. Direct service and support will be provided to clients in areas such as daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual’s needs. What You Will Do: Create individualized support and recovery plans with clients and professionals based on assessments. Support clients in building independence by teaching daily living skills (e.g., hygiene, medication, cooking, budgeting). Coordinate care by connecting clients with social, medical, and community support. Provide crisis intervention and help develop crisis plans. Represent the program professionally with clients, families, and community partners. Mediate conflicts and help clients learn problem-solving skills. Organize or support social activities to reduce isolation. Maintain accurate client records and documentation. Work collaboratively with the team, contribute ideas, and support new staff or volunteers. Provide case management and connect clients with services like housing and ID support. Help clients access financial supports such as Ontario Works (OW) and Ontario Disability Support Program (ODSP) or other financial resources as required. Other duties as assigned by Program Manager and/or designated supervisor. What You Bring: Post-secondary education in Social Services / Human Services or equivalent field. Minimum 3 years’ working experience with client population group. Understanding of challenges faced by Adults with mental illness, addictions, physical health concerns, and homelessness etc. Proficient in intake assessment, crisis intervention, advocacy, and problem-solving to support client needs. Ability to build therapeutic relationships and collaborate with clients on goals. Familiarity with community resources and professional services. Strong teamwork skills in a multidisciplinary environment. Strong interpersonal, communication, and counselling skills to effectively engage clients in sensitive areas such as life skills, healthy living, and relationships. Solid computer literacy for maintaining records, files, and reports. Effective analytical, organizational, and time management skills for coordinating client support and program activities. Demonstrates cultural competency, with an understanding of diverse backgrounds and inclusive practices. Must have a valid driver’s license, insurance, and a reliable vehicle to travel within the region.
