Medication Management Clinical Pharmacist Position Summary The Medication Management Clinical Pharmacist serves as a clinical and strategic pharmacy resource for hospice organizations, providing medication management expertise, formulary guidance, and pharmacy benefit consultation. This role is responsible for conducting comprehensive medication reviews, identifying opportunities to optimize therapy, and providing evidence-based recommendations that improve patient outcomes and support cost-effective medication use. The pharmacist collaborates closely with hospice leadership, medical directors, physicians, nurses, and interdisciplinary care teams while serving as a trusted advisor on medication management, pharmacy utilization, and clinical best practices. Essential Responsibilities Clinical Medication Management Conduct comprehensive medication reviews for hospice patients, identifying appropriateness, drug-drug interactions, contraindications, adverse drug events, and therapeutic duplication. Review high-cost medications and recommend cost-effective alternatives where appropriate. Answer medication-related questions and provide evidence-based recommendations to optimize therapy, improve symptom management, support formulary alignment, and enhance patient care. Leadership and Clinical Consultation Conduct monthly and quarterly executive reviews with hospice leadership to analyze medication utilization, prescribing trends, pharmacy spend, and clinical outcomes. Present reports, dashboards, key performance metrics, and strategic recommendations to support medication management, quality improvement, and cost-containment initiatives. Serve as a pharmacy consultant to leadership by providing guidance on regulatory requirements, industry trends, and hospice medication management best practices. Quality, Compliance, and Education Ensure compliance with federal, state, and accrediting agency standards related to medication management. Support quality assurance and performance improvement initiatives. Develop educational materials and deliver clinical presentations on hospice pharmacotherapy, medication management, and formulary strategies. Foster collaborative relationships with hospice leadership, providers, pharmacies, and medication management partners. Qualifications Education Doctor of Pharmacy (PharmD) degree from an accredited college or school of pharmacy. Active, unrestricted pharmacist license in good standing. Experience Minimum of 3–5 years of clinical pharmacy experience. Experience in hospice, palliative care, managed care, pharmacy benefit management, long-term care, or a related healthcare setting preferred. Demonstrated experience conducting comprehensive medication reviews and providing clinical recommendations. Knowledge, Skills, and Abilities Advanced knowledge of pharmacotherapy, medication management, and evidence-based clinical practice. Strong analytical and data interpretation skills. Ability to communicate complex clinical and financial information to executive audiences. Excellent written, verbal, and presentation skills. Strong consultative and relationship-building abilities. Proficiency with healthcare technology platforms, reporting tools, and Microsoft Office applications. Ability to work independently and effectively manage multiple priorities.
