Responsibilities include: * Conduct comprehensive health assessments, diagnose and treat common acute and chronic conditions within the scope of practice. * Provide timely episodic care for acute issues, ensuring efficient assessment, management, and appropriate follow-up. * Order, interpret, and evaluate diagnostic tests and prescribe appropriate medications. * Develop and implement individualized care plans in collaboration with patients and their families. * Provide patient education and counseling on disease prevention, wellness promotion, and health maintenance. * Collaborate effectively with our team of Nurse Practitioners and Family Doctors to ensure optimal patient care and continuity of services. * Maintain accurate and up-to-date medical records using our electronic health record system. Qualifications: * Current and valid Nurse Practitioner license in Ontario. * Demonstrated clinical competence in providing primary care services to patients across the lifespan. * Strong interpersonal and communication skills, with the ability to build rapport with patients and collaborate effectively within a multidisciplinary team. * Proficiency in using electronic health record systems.
