PRN Occupational Therapist (OT) – Skilled Nursing & Rehabilitation Meadowview Rehabilitation & Nursing Center – Lafayette Hill, PA Join Our Team and Help Patients Regain Independence! Meadowview Rehabilitation & Nursing Center is seeking a compassionate and dedicated PRN Occupational Therapist (OT) to join our in-house rehabilitation team in Lafayette Hill, Pennsylvania . This flexible PRN opportunity is ideal for an OT who is passionate about delivering high-quality, patient-centered care in a Skilled Nursing and Rehabilitation setting. As a valued member of our interdisciplinary team, you'll help residents maximize their independence, improve daily function, and achieve their rehabilitation goals. Responsibilities Evaluate residents and develop individualized occupational therapy treatment plans. Provide skilled occupational therapy services to short-term rehabilitation and long-term care residents. Assess functional abilities, activities of daily living (ADLs), upper extremity function, cognition, and home safety needs for discharge planning. Collaborate with physicians, nursing staff, therapy team members, and families to provide coordinated patient care. Educate residents and caregivers on adaptive equipment, safety strategies, and therapeutic exercises. Complete accurate and timely documentation using Net Health and PointClickCare (PCC). Maintain compliance with all federal, state, and facility regulations. Qualifications Current Occupational Therapist (OT) license in the Commonwealth of Pennsylvania. Graduate of an accredited Occupational Therapy program. Skilled Nursing Facility (SNF) or Long-Term Care experience preferred, but not required. Experience with Net Health and PointClickCare (PCC) is a plus. Strong communication, clinical, and organizational skills.
