Key Responsibilities * Deliver direct patient care in diverse healthcare settings, including acute care, assisted living, and medical offices. * Administer medications and treatments as prescribed by healthcare providers. * Monitor and assess patient conditions, focusing on pain management and symptom control. * Collaborate with interdisciplinary teams to create and implement individualized care plans. * Assist in case management and coordinate care for patients with complex medical needs. * Accurately document patient information in electronic health record systems such as Cerner. * Educate and support patients and their families regarding healthcare needs and care plans. Experience & Qualifications * Minimum of 1 year of experience as a Registered Practical Nurse (RPN). * Experience in acute care, dementia care, hospice care, or other specialized areas is an asset. * Strong skills in medication administration and familiarity with various administration routes. * Knowledge of pain management protocols and best practices. * Familiarity with medical office procedures and healthcare documentation. * Demonstrated ability to provide compassionate, patient-centered care. * Exceptional communication skills for effective interaction with patients, families, and healthcare teams. Requirements * Current registration with the College of Nurses of Ontario (CNO). * Completion of an RPN certificate program.
