Job Description Job Description Social Worker - Full-Time (Flexible Schedule) Bluewater Area Family Health Team - Zurich, Ontario About the Role Join a welcoming interdisciplinary team including physicians, nurse practitioners, nursing staff, and another Social Worker Work collaboratively to support patients with complex and evolving needs Contribute to thoughtful, coordinated, team-based care What You'll Do Provide counselling and psychosocial support across the lifespan Support patients with: Mental health concerns Addictions Chronic illness Life transitions Balance both brief, solution-focused support and longer-term therapeutic relationships Collaborate with team members on care planning and shared decision-making Participate in case discussions and team meetings Contribute to the development of programs and services that reflect community needs Why Rural Practice Build meaningful, ongoing relationships with patients and families Work across a broad scope including individuals, couples, and families Be part of a close-knit, supportive, and approachable team Have regular opportunities for consultation and collaboration Experience variety and depth in your day-to-day work What to Expect A busy and sometimes unpredictable environment A strong team approach-you will not be working in isolation Emphasis on sustainable care for both patients and staff What We Offer Full-time position with flexible scheduling Four (4) weeks vacation Two (2) weeks personal days Paid professional development days Education allowance Participation in the Healthcare of Ontario Pension Plan (HOOPP) What We're Looking For Registered with the Ontario College of Social Workers and Social Service Workers Strong collaboration and teamwork skills Effective communication skills Comfortable supporting a wide range of patient needs Ability to adapt in a dynamic, team-based rural environment Company Description Bluewater Area Family Health Team is a rural primary care clinic in Zurich, Ontario, providing team-based care through a collaborative group of physicians, nurse practitioners, nurses, and allied health professionals.
