Social Worker / Geriatric Care Manager (MSW/LMSW) Help Seniors Age with Dignity and Independence For more than 100 years, our organization has been dedicated to improving the lives of older adults. Through a unique private endowment established in 1925, we provide financial assistance, care management, advocacy, and personalized social work services that help seniors remain safe, independent, and connected to their communities. We are seeking a compassionate, organized, and self-motivated Social Worker / Geriatric Care Manager to join our growing team. This is an excellent opportunity for a social worker who is passionate about working with older adults, enjoys building meaningful relationships, working independently in the field, and collaborating with an experienced and supportive team Key Responsibilities Manage a caseload of older adult clients (low 20s max) Conduct in-home visits and ongoing client assessments Develop and monitor individualized care plans Advocate for clients across healthcare, housing, and social service systems Assist with access to benefits, entitlements, and community resources Collaborate with families, providers, and support networks Maintain accurate and timely documentation Participate in weekly team meetings and case conferences Required Qualifications LMSW or LCSW preferred (MSW considered with relevant experience) Experience in care management, case management, or working with older adults Valid driver’s license and reliable vehicle required Ability to conduct field-based home visits throughout NYC and surrounding areas Strong communication, organization, and documentation skills Benefits / Highlights Hybrid schedule (1 in-office day, remainder remote/field-based) Low caseload (max low 20s) Mileage reimbursement for field visits Stable, mission-driven organization with strong retention Tight-knit team environment (team of 6 social workers) Direct hire with full benefits Work Environment Wednesday required in-office (9 AM–5 PM) for team meetings Remaining days include remote work and field visits Minimum one in-person visit per client per month Skills for Success Care Management & Client Advocacy Geriatric Social Work Independent Field Work & Time Management Relationship Building with Clients & Families Resource Coordination & Benefits Navigation Documentation & Case Tracking Communication Across Multidisciplinary Teams Problem-Solving & Critical Thinking Adaptability in Community-Based Settings Professionalism & Client-Focused Approach
