Develops a plan of care to promote optimum function and safety as documented in the clinical record Incorporates patient goals into the plan of care ```{=html}

Provides skilled speech therapy treatment in the home, reassessing the clients status during each visit and ensuring quality of care as documented in the clinical record ```{=html}

Coordinates patient care with other clinical team members ensuring optimal patient care and communication as noted in the clinical record ```{=html}

Follows hospital exposure control plans/blood borne and airborne pathogens ```{=html}
``` -
Maintains confidentiality of all hospital and patient information at all times. Follows HIPAA regulations and policies ```{=html}

Regular and predictable attendance is an essential job function ```{=html}

Communicate with patients, families and co-workers in person, in writing and on telephone **Minimum Qualifications:** -
Masters degree in Speech Pathology required ```{=html}

Current State of Idaho Speech Language Pathologist Licensure required OR temporary SLP Provisional Permit ```{=html}
``` -
Certification with the American Speech and Hearing Association
(ASHA) required OR will obtain upon completion of Clinical
Fellowship Year
Minimum of 1-year experience with adult and or pediatric neurologically impaired patient population preferred.
