All Nations Hope Network Inc. Position Type: Full-Time SummaryAll Nations Hope Network Inc. is seeking a compassionate and culturally grounded Navigator to support Indigenous individuals living with or at risk of HIV, AIDS, hepatitis C (HCV), sexually transmitted infections (STIs), and other blood-borne pathogens.The Navigator provides trauma-informed, client-centered support by helping individuals access health care, treatment, housing, mental health, addictions, cultural, and community services. The role also provides health education, conducts community outreach, promotes harm reduction, and encourages participation in Indigenous cultural activities and wellness programming.Key ResponsibilitiesProvide one-on-one client support, advocacy, and system navigation.Connect clients with health, social, cultural, and community resources.Conduct community outreach and build relationships with individuals at risk.Deliver education on HIV, HCV, STIs, blood-borne pathogens, and harm reduction.Support clients in accessing Indigenous cultural teachings, ceremonies, and wellness activities.Maintain accurate client documentation and prepare monthly activity reports.Collaborate with community agencies and health care providers to improve client outcomes.Skills & QualificationsDiploma or degree in Social Work, Indigenous Social Work, Human Services, Community Health, Addictions, or a related field, or an equivalent combination of education and experience.Knowledge of HIV, HCV, STIs, harm reduction, and Indigenous health and wellness.Strong communication, advocacy, and client support skills.Experience with case management, referrals, and community outreach.Ability to build positive relationships with clients and community partners.Excellent organizational, documentation, and time management skills.Ability to work independently and as part of a team.Proficiency with Office 365Valid SK Class 5 Driver's License
