*Job Title: *Program Coordinator Main Purpose of the Job: The Program Coordinator is responsible for the day-to-day operations of the assigned client programs. The position will ensure staff members are providing the highest level of quality care and service provision to the clients and the community. The Program Coordinator provides trauma informed support respecting client care to all staff members, while completing touch base meetings and annual reviews specifically to the staff members they are supporting. The Program Coordinator is responsible to provide guidance and day to day support to staff members and clients while providing daily updates to the Program Lead to ensure the delivery of optimal client care services. The Program Coordinator is flexible within their role based on the needs of Rowan House Society, which may consist of taking a more direct role in supporting clients and the programs. Additionally, the Program Coordinator will support the staff members at the various locations such as the Emergency Shelter, Community Program Offices or in the community. *Reports to: *Program Lead Duration: Maternity Leave Coverage, 12-18 months Direct Reports: Community Programs ( Outreach Program, Preventative Education Program, Legal Resource Navigator, and Safe at Home Program) Duties and Responsibilities: * Ensure the daily operational requirements are met for the assigned Client Programs * Provide crisis case management and supportive planning from a trauma informed approach while maintaining client files with integrity and accuracy. * Connect and provide support for the assigned programs. * Support and facilitate day-to-day programming, communicate any program gaps, and collaborate with Program Lead in executing plans to improve client care delivery services. Ensure staff members, volunteers, and practicum students are integrating program objectives in their practice for optimal client service delivery. * Ensure client and staff grievances are brought forward to the attention of the Program Lead in a timely manner. * Ensure Dual Relationships are brought forward to the attention of the Program Lead to solidify the required process. * Engage staff members in identifying their inherent strengths and to utilize a solution-focused approach to their work. Discuss and review staff members? annual review goals and their progress. * Responsible for reviewing client?s file on Outcome Tracker and providing staff members with feedback to enhance their case note practice. Ensure case notes are reviewed and locked, review intake and discharge information, ensure client goals are completed, Critical Incidents have been reviewed and advised to the Program Lead, and ensure there is no missing data on client?s file. * Ensure Assessment tools are completed for each client unless the client declines. Ensure that a safety or accountability plan is completed in alignment with the Assessment tool, or based on the needs the client identifies. * In collaboration with the Program Lead, support the hiring process of new staff members or practicum students by participating in the interview, the orientation, and the onboarding processes. Additionally, this includes the discontinuation of a staff member or practicum student, which involves ensuring all documentation is completed and all devices are returned. * Responsible for performance management processes with staff members which includes advising Program Lead and Finance and Administration Manager on the next steps. * Support and participate in the On-Call Rotation and provide coverage for shifts not covered, as well as ensure a summary and outcome of the call is documented. * Other duties as required and determined by the Program Lead.
