What You'll Do Provide routine telehealth visits with Medicare patients (E/M and Chronic Care Management) Create and manage personalized care plans for ongoing conditions Collaborate with our integrated care team (nurse case managers, NPs, CHWs) Ensure thorough documentation in our EHR and compliance with quality measures Engage patients in preventive care to reduce avoidable complications Why Belle, Why Now Healthcare is shifting homeward—and seniors deserve better.
